Results from this thesis describe new aspects of knee joint biomechanics in individuals with knee OA and provide a rationale for the use of measures of dynamic alignment in the study and treatment of knee OA.

Several interventions for knee osteoarthritis (OA) are aimed at altering lower limb alignment to reduce joint load during dynamic activities such as walking. The planning of these interventions, however, typically focuses on measures of static alignment that may not adequately represent dynamic situations. Therefore, the purposes of this thesis were to: (1) highlight limitations in using measures of static alignment, and (2) introduce and evaluate novel measures of dynamic alignment for possible use in interventions for patients with knee OA.

Five studies involving radiographic measures and three-dimensional gait analyses obtained from patients with knee OA were performed. Findings suggested that the static mechanical axis angle was affected by changing foot positioning during radiograph acquisition. Although the static mechanical axis angle explained a significant portion of variance in the external knee adduction moment during gait---an indirect measure of dynamic medial compartment knee joint load---the addition of gait kinematics such as toe-out and lateral trunk lean angles increased the amount of explained variance substantially. Negative correlations between the knee adduction moment and toe-out and trunk lean angles suggested these kinematics have the potential to reduce knee joint load during gait. Two new measures of dynamic lower limb alignment based on gait analysis were examined: (1) a measure obtained from reflective markers used to quantify hip, knee, and ankle centres, termed marker-based alignment, and (2) the perpendicular distance between the knee joint centre and the ground reaction force vector, termed the frontal plane lever arm. These measures of dynamic alignment were highly correlated with the static mechanical axis angle and the knee adduction moment, indicating they were valid measures of alignment and were associated with knee joint loading. However, only the dynamic lever arm measure was consistently correlated to toe-out and lateral trunk lean. Lastly, all measures of static and dynamic alignment and knee joint load were significantly improved after high tibial osteotomy surgery.
