Purpose/Hypothesis. Women with primary osteoporosis, or bone demineralization due to aging and menopause, are at increased risk for frailty because these women experience increased loss of muscle and bone tissue as compared to healthy peers. It is possible that early post-menopausal women with low bone mineral density (BMD) may present with early markers for physical frailty. The purpose of this study was to determine if early post-menopausal women with low BMD present with decreased physical performance in dynamic balance, strength, and gait. Subjects/Sample. Basic inclusion criteria were women aged 50-65; 3-10 years post menopause; with a bone density scan in the past 2 years. A total of 54 women, 31 with low BMD and 23 with healthy BMD, were included in this study. Materials/Methods. Subjects were asked basic demographic information and were administered the Stanford 7-Day Physical Activity Questionnaire. Subject height and weight were measured to calculate BMI. Dynamic balance was assessed via the timed backwards tandem walk test. Strength was assessed via handheld dynamometry of quadriceps isometric force production. Fast gait speed step length was assessed via the GaitMAT II. Analysis. An ANCOVA was performed for all variables treating age, body mass index, and physical activity level as covariates. Results. The low BMD group demonstrated a tendency towards decreased performance in all physical performance testing. Fast gait speed step length was significantly shorter (p ≤ .05) in the low BMD group. Conclusions. Early post-menopausal women with low BMD may demonstrate subtle changes in physical performance. Temporal-spatial gait characteristics may change prior to balance and strength measures in early post-menopausal women with low BMD.
