Geriatric self-neglect is the form of elder abuse most frequently reported to Adult Protective Services in the United States, accounting for about half their referrals. It is defined as the consequence of not providing oneself with appropriate food, medical care, clothing, shelter, or the goods and services necessary to maintain physical, mental, and emotional well-being and general safety. Cases of self-neglect present challenges for service providers because many seniors who need care, often refuse it. The question of whether these elderly represent victims of self-neglect who require intervention or simply individuals invoking their right to self-determination is an ethical dilemma and one that continues to plague practitioners and researchers alike. Over the past five decades, very little research has been conducted in this field.

This study investigated the factors associated with self-neglect in community-dwelling older adults. Its purpose was to test a conceptual model of self-neglect, identify potential risk factors for substantial to severe self-neglect, and examine the relationship between substance abuse and self-neglect. This descriptive study employed a cross-sectional, survey research design that was exploratory in nature. In total, 106 community-dwelling subjects aged 60 and over were sampled and interviewed from seven low-income apartment buildings in Cuyahoga County. The response rate was 72.1%.

Results indicate that male gender, no medication use, moderate to high medication misuse, and problems with eyesight were significant predictors of self-neglect. The regressors (male gender, no medication use, smoking, moderate to high medication misuse, problems with eyesight, alcohol problem) taken together explained 31% of the variance in the dependent variable (adjusted R2 was .27). Male gender was the only statistically significant potential risk factor for substantial to severe self-neglect. These findings point to the significance of investigating substance use and abuse in association with self-neglect, especially in older males. Given the increasing numbers of self-neglect cases and difficulties in managing this condition, additional methodologically rigorous research is needed, similar to the methods in this study that utilized clear conceptual definitions, standardized measures, a systematic sampling procedure, and multivariate analyses.
