When physicians complete the official training process such as residency/fellowship, they are given the title of attending. Once the first-year attending acquires the highest status granted to a physician, they are expected to possess all the requirements that are necessary when caring for patients. Many social science studies focus on the socialization of medical students and residents with little attention given to the transitional year of the first-year attending. Some researchers assume that because physicians have finished training, to do further research into the socialization process of a first-year attending is moot. In this ethnographic study conducted in a prestigious academic pediatric hospital, a series of focus group interviews were performed. After analysis of the data, a two phase process was observed while completing the transitional year from residency/fellowship into attending status. The first stage of the first-year attending socialization process is entitled style of doctoring. Style of doctoring is categorized as when to wear the white coat, communicating with families, and designing a treatment plan. Once the aforementioned decisions are made the first-year attending must then enter the more challenging stage of training which is referred to as acquiring an authoritative role. Thus by proceeding to this next part in the attending's socialization process, there are three consequences that accompany this new authoritative role. These consequences are as follows: uncertainty, malpractice, and personal life. Throughout the body of this study there are several patient stories that were provided by first-year attendings and ethic committee meetings. The results of this research demonstrate that there are unique sociological issues encountered by first-year attending physicians. By exploring the first-year attending's socialization process and the various ethical dilemmas faced when caring for pediatric patients; medical educators will expand their body of knowledge and improve patient care.
