In an effort to save money on prescription drugs, some consumers have purchased and expressed intentions to purchase medications from outside the U.S. via the Internet. While saving money often is cited as a reason for purchasing prescription drugs via non-U.S Internet pharmacies (drug importation), the availability of medications without a legitimate prescription is also a motivation. Although considered unlawful, the legality of prescription drug importation, with or without using a prescription obtained from a physician, remains unclear from the perspective of the consumer. Consumers may be aware of the illegality of drug importation, but they may justify conducting importation based on moral reasons. Using an extended version of the Theory of Planned Behavior (TPB) this study examined consumers' intentions to engage in two illegal and ethically questionable behaviors: (1) purchase a prescription drug from a non-U.S. Internet pharmacy using a prescription obtained from a physician and (2) purchase a prescription drug from a non-U.S. Internet pharmacy without using a prescription.

A cross-sectional design was used to collect data from an online consumer panel. Two versions (A and B) of the questionnaire were developed. After online administration of the questionnaires, complete and usable responses were obtained from 332 respondents for Version A and 294 respondents for Version B.

Structural equation modeling was used to test the hypotheses. Results indicated that consumers' intentions to purchase prescription drugs from non-U.S. Internet pharmacies using a prescription were influenced by subjective norms, perceived moral obligation, perceived unfairness, perception of costs and quality of drugs available outside the U.S. Consumers' intentions to purchase prescription drugs without using a prescription were influenced by subjective norms, perceived moral obligation, perceived unfairness, and perceptions of costs of drugs available in the U.S. Multiple-group analysis showed no differences between the two models.
