The purposes of this thesis were to describe the use of prophylactic antibiotics in terms of an effective first prophylactic dose (EFPD), and surgical site infection (SSI); and relate them to hospital and patients factors.

A proven method to reduce the incidence of surgical site infection is to administer appropriate prophylactic antibiotics within 2 hours of the incision, in the right dose, with the correct drug, using the correct route. However operational deficiencies compromise this simple process. To address these operational deficiencies an understanding of the process must first be accomplished.

Over a 4-year span (1994--1998) at Kingston General Hospital, 4441 patients were retrospectively extracted from the database. They were divided into five groups according to five procedure categories: cardiothoracic, colonic, gynaecologic, orthopaedic, and vascular. For each procedure, forward stepwise multiple logistic regression (MLR) was applied to relate key hospital and patient variables to the EFPD outcome. Due to the small number of SSI cases in the data set, only one stepwise MLR model was developed to identify possible risk factors. Surgical procedure was included in the model as a stratification factor to account for the differences between procedures. (Abstract shortened by UMI.)
