This study examines the interactional practices through which physicians engaged in medical peer review manage the conflicting pressures of bureaucratic control and professional autonomy. Using the methods of conversation analysis, this study describes the organization of the review as a socially constituted form of institutional interaction; it also examines the structure, organization, and consequentiality of particular activities within the review, such as the initiation of case discussion and the announcement of decisions. The study demonstrates that, contrary to traditional sociological understandings of the nature of bureaucratic and collegial control, the threat of the bureaucracy to the collegial relationship is frequently managed through practices that maximize the bureaucratic nature of the review and minimize its collegial aspects. Thus, disagreement over the treatment of patients may be mitigated by assuming a bureaucratic stance toward the review.
