The purpose of the study was to examine the phenomenon of choice of medical specialty at the level of the individual medical student to better understand one aspect of the physician work force problem. The influences of student characteristics, institutional characteristics, and environmental demands on specialty decisions were probed.

A conceptual model of specialty choice was developed for the study, grounded in social learning and professional socialization theories. The model provided a means of accounting for the accumulating specialty choice literature and as a preliminary suggestion for how these medical student decisions come about. The findings of this study suggested the need for greater detail which was added to the conceptual model.

Three student subjects from each of medical school years one, two, and three were selected for the study based on their expressions of initial interest in family medicine or another primary care specialty. Data were generated from interviews, focus groups, participant observation, and document reviews. Thematic analyses of transcribed fieldnotes seeking convergence of perspectives were conducted.

Major findings to emerge included the suggestion that specialty choices of students with family medicine interest upon matriculation to medical school may be influenced more by their pre-matriculation experiences than their post-matriculation experiences. Distinct decision making styles of students from the three medical classes were reported and described. Finally, the study reported and described the role of the external environment as promoting choice of primary care, and specifically family medicine.
