Risk of AMI and sudden cardiac death, caused by clot formation in the coronary arteries, is increased during the one-hour period following an acute bout of physical exertion. Therefore, the purpose of this study was to determine the changes in clotting and fibrinolytic activity during the one-hour period following an acute bout of sub-maximal exercise at a specific relative intensity of exercise. Ten apparently healthy males (23.5 yr.) ran at 70–75% VO<sub> 2</sub> max and walked slowly at 1.2 mph for 30 minutes in random order. Subjects reported in the morning between 07:00 and 10:00 (10–12 hrs, post-absorptive) and rested 15–30 minutes. Venous blood was obtained at rest, immediately (within 2–4 minutes) after the run or walk and every 20-minute during the one hr recovery. Plasma was analyzed for indicators of clotting [activated partial thromboplastin time (APTT) & factor VIII activity (FVIII)] and fibrinolytic activity [t-PA antigen (tPA) & D-dimers (Dd)]. A 2 x 4 repeated measures ANOVA was used to analyze the data.

Pre-treatment values (running and walking) were similar for all the dependent variables and not included in the statistical analysis. The post-exercise main effect means of APTT for the running and walking condition were 30.78 and 32.99 sec, respectively [<italic>F</italic>(1,9) = 22.48, <italic>p</italic> = 0.0011]. The post-exercise main effect means of FVIII for the running and walking condition were 195.85 and 128.82%, respectively [<italic>F</italic>(1,9) = 21.79, <italic>p</italic> = 0.0012]. There was a significant time x treatment interaction for changes in tPA [<italic>F</italic>(3,54) = 23.41, <italic> p</italic> < 0.0001]. A quadratic trend with negative slope was present for changes in tPA after the run, while changes after walking showed a linear trend with zero slope. The post-exercise main effect means of Dd for the running and walking condition was 68.36 and 45.31 ng/ml, respectively [<italic> F</italic>(1,9) = 45.68, <italic>p</italic> = <0.0001]. Increased Dd suggests the presence of <italic>in vivo</italic> clot formation and fibrinolysis. There was no main effect of time, or time x treatment interaction for changes in APTT, FVIII and Dd (<italic>p</italic> <math> <f> ≥</f> </math> 0.28). This data suggests that running at 70–75% VO<sub>2</sub>max results in elevated clotting and fibrinolytic activity which lasted for one-hour post-exercise period. However, sustained clotting activity in conjunction with declining tPA could favor clot formation during this period.
