The physical and mental health of school-age children are of growing concern in US schools. Thirty-one percent of children age 6--12 years are overweight or at risk for being overweight. The serious long-term consequences of being overweight in childhood include adult obesity, type 2 diabetes, heart disease and sleep apnea. Twenty-one percent of children age 9--17 years exhibit signs and symptoms of mental illness, but only 25% of these children receive mental health services. A potentially serious consequence of mental health illness is suicide, the third leading cause of death in adolescents. The purpose of this study was to understand the value of a supervised physical activity program provided by a school-based health center for children with physical and emotional concerns. The experiences of 20 fourth and fifth grade students were described through a combination of quantitative and qualitative measures. The findings were integrated in three case studies to reveal the clarity and relevance of the experience through the voices of the children, their parents and their teachers as well as paint a more holistic picture of the children involved with the program.

520 $a The quantitative one group pretest-posttest design included measurements of BMI, waist circumference, blood pressure, depression as measured by the Child Depression Inventory, and well-being as measured by the Well-being Picture Scale for 20 fourth and fifth graders who participated in an after-school physical activity program, four days a week for four months. While half of the children attended more than 50% of the sessions, attendance was poor for the remaining half. All but one of the children in the program were overweight or obese at the beginning of the program. Overall, quantitative measures for weight status did not improve at the end of the program. Twenty percent of these children were at risk for depression and four identified that they thought about killing themselves at the beginning of the program. Alarmingly, two children expressed that they wanted to kill themselves at the end of the program.
520 $a The qualitative phenomenological design included interviews with seven children, eight parents, four teachers and two health professionals. Although the program was viewed favorably by those interviewed, the students?? stories confirmed that the daily life of fourth and fifth graders is complex. The four themes which emerged from these interviews were: (1) what the program meant to the children, (2) it's more than a weight issue, (3) the need to belong in school, and (4) the importance of school connections. Some of the children in the program struggled with additional health challenges (such as asthma, strep throat, and muscle weakness), academic problems, and the effects of poverty. This program began to address the need to belong and feel connected with other peers while attending school. Likewise, the findings indicate that initial steps to connect the health and educational resources required to meet the comprehensive needs of these children were accomplished through the presence of a school-based health center.
520 $a Implications for further study include efforts to improve attendance in the activity program and the incorporation of new recommendations (limited sweetened beverages and decreased screen time). Exploration of the prevalence and meaning of suicidal ideation in childhood emerged from the findings as an urgent need. The potentially powerful and important role of the school community in the prevention and treatment of physical and mental health challenges in school-age children was confirmed and should be examined on a broader level.
