The purpose of this study was to explore, describe, and interpret narratives of older black women regarding interpersonal processes of care during visits to their physicians for the treatment of hypertension. Sixteen older black women (ages 66-81) participated in face-to-face interviews. Narratives were analyzed to understand interpersonal processes, especially of age, race, or gender biases potentially contributing to health disparities. Understanding of the self-identities of these older black women was gained.

The Interpersonal Processes of Care (Stewart, Napoles-Springer, and Perez-Stable, 1999) guided data collection and analysis and revealed high quality physician communication, decision-making, and interpersonal style in most cases. Theories of Narrative, Black Feminist/Womanist, and Communicative Action framed data analysis and interpretation, leading to the discovery of empowerment as a pervasive thematic element in the narratives. Two themes emerged within the interpersonal processes of care: Claiming Power, and Things that are important to me. Among things that are important to me were God has the last say so, concerned about my, family, and serious about my health.

Five plot types for interpersonal processes of care within the patient/physician relationship were identified: shared-voice, quest, restitution/redemption, and chaos, and victim narrative. The shared-voice narrative emerged as a particularly unique narrative type in these data. Shared-voice resulted when patient and physician verbal exchange integrated both the lifeworld of the older black woman and medical practice. The most powerful subplot was the effect of empowerment on the patient/physician relationship, the woman's self-identity and entire lifeworld.
