The purpose of this quantitative study, using a randomized controlled research design, was to examine whether an educational intervention influenced physician empathic expression with patients. The study was premised on the research that poor physician communication specific to the underutilized core skill of physician empathy has contributed to adverse health outcomes. The study relied upon three measures to make comparisons about physician empathic behavior based on outside observer measures and patient self-reports. The research accessed a random sample of archival data using original audiotapes of physician-patient encounters to measure physician-expressed empathy using a hierarchical outside observer measure of physician empathy. The educational intervention included a series of communication workshops that employed didactic and experiential teaching modalities, skills practice sessions and individual follow-up coaching sessions. The results strongly supported the claim that training made a significant difference in physician empathic expression during patient interactions demonstrated by both outside observer measures of global ratings and hierarchical physician ratings of physician empathy. The differences in global empathy scores in the physician training group from baseline to follow-up improved by 37% and up to 51% in hierarchical scores of physician empathic expression. The significant effects using different measures of empathy strengthens the findings regarding the efficacy of the intervention. The limited correlation between the measures suggests that empathy is multi-dimensional. These findings, although within the context of medical education and training, has significant implications for program design and development for other leadership training and professional development.
