The concept of empathy in the physician-patient relationship has received much attention in the literature. However, a theoretically- and empirically-based measure of a physician's communication of empathy to a patient has not been developed to date. Consequently, this dissertation research attempts to remedy this by: (1) Examining patient perceptions of empathic communication in the physician-patient encounter. (2) Developing a measure of empathic communication for the physician-patient encounter---the Empathic Communication Coding System. (3) Applying this measure of empathic communication to encounters between medical students and simulated patients as well as those between physicians and patients.

This dissertation proposes that empathy in the physician-patient encounter is more than sharing another's emotion, and explicates several types of behaviors that characterize the communication of empathy. The Empathic Communication Coding System takes these behaviors into account, providing a hierarchical coding scale for physician responses to patient-created empathic opportunities.

Further, this dissertation investigates the relationships between characteristics of the patient-created empathic opportunities and the resulting physician empathic communication. Other physician and patient characteristics are explored to determine their fit as predictors and outcomes of physician empathic communication.
