Retinal detachment resulting from macular pucker repair can be a serious post-operative complication. Management of post-op complication must first start with the ability to identify a problem. The nurse practitioner (NP) is charged with knowing postoperative complications for patients. Since NPs are part of the surgical team, and are seeing some patients post-operatively, they are an excellent source for post-op adverse reactions. The 25 gauge and the 20 gauge vitrectomy systems have both been documented in literature to be capable in correcting retinal problems. These systems have also been associated with post-op retinal detachments. The nurse practitioner's roles, in providing care post-operatively, and identifying complication should not be overlooked. A retrospective descriptive study was conducted with review of 300 charts of patients with macular puckers, to examine the difference in the rate of retinal detachments complications following the use of the 20 gauge system and the 25 gauge system. The setting was a large, urban, 555-bed hospital that offered many different types of services, which includes eye surgery. This study found that there was a difference (t = 3.5%--0.6% = 2.9%) in the proportions of detachments between the 20-gauge and the 25-gauge vitrectomy systems with a significance of (p = 0.0075).
