The proportion of young sexually active men in Vietnam who have sex with female sex workers (FSWs) is estimated to be 10-30%---approximately 1.5-4.5 million men. Knowing the HIV prevalence and sexual behaviors of these men is important for understanding public health implications and planning interventions. Between April and May 2007, 292 male clients visiting sex venues were interviewed on-site at sex venues in the Thien Loi area and Do Son Beach of Hai Phong, Vietnam in a cross-sectional survey using two-stage time-location cluster sampling. The interview was administrated by CD player and earphones in an anonymous manner, followed by a blood draw or HIV rapid oral fluid testing. MV, syphilis, and HS V-2 prevalence among clients of the Thien Loi area were 7.04%, 2.9%, and 18.5%, respectively, and among clients of Do Son Beach, 3.62%, 0%, and 12.2%, respectively. Seventy percent had consumed alcohol before their most recent sex encounter with a FSW, 6.8-20.7% had ever used recreational drugs, 20-25% reported sign(s)/symptom(s) of STIs in the past year, and 2-11% reported having had sex with other men. While there was a fairly high level of reported consistent condom use with FSWs (91.8-94.4%), there was a low prevalence of consistent condom use with wives (33%), girlfriends (53.4%), and irregular (casual) sex partners (74.4%). Over half (55.8%) of the male clients were potential bridgers (men who have sex with both high-risk (FSWs) and low-risk (regular) partners during the same time period, and always use condoms with high-risk partners but not with low-risk partners) or active bridgers (do not consistently use condoms with either type of partner). They had a significantly higher herpes simplex type 2 (HSV-2) prevalence (21% & 33% vs. 8.7%). HIV seropositivity was 4.4-fold (95% CI: 1.1-17.1) higher among those who were HSV-2-positive. Clients of FSWs may be playing a major bridging role in transmitting HIV and STIs.

The adjusted odds ratios (ORs) of being HIV- and/or syphilis-seropositive were 5.17 (2.1-12.7) among clients ever having used drugs, 2.96 (1.1-7.9) among regular customers of the sex venues, and 2.33 (1.2-4.6) among clients experiencing condom slippage/breakage in the past month. There was a synergistic interaction between drug use and condom breakage/slippage. Potential bridgers were 1.8-fold (1.95-3.03) more likely to feel that condom use was not necessary with one's wife/girlfriend, and active bridgers, 4.34-fold (1.35-14.28). Intervention should emphasize the importance of always using condoms with low-risk women, as it is the only effective way to prevent HIV and STIs. Condom use skills should be taught to the general population, the quality of condoms should be carefully controlled, and anti-drug programs implemented that target sex workers and their clients.
