Patients with Type II diabetes may struggle to manage blood glucose levels independently. There was no collaborative communication strategy at the project site to provide diabetes education and support, so an evidence-based solution was sought. The purpose of this quality improvement project was to determine if the implementation of American Diabetes Association's (ADA) Standards of Care in Diabetes - 2023, Standard 5: Facilitating Positive Health Behaviors and Well-being to Improve Health Outcomes on digital coaching and digital self-management would impact HbA1c among adult patients with type II diabetes. The project was piloted over a 12-week period in a rural West Virginia primary care setting. Dorothea Orem's self-care deficit theory and William Edwards Deming's plan-do-check model provided the project's scientific underpinnings. Data were collected on HbA1c levels from the electronic health records on N = 33 patients at baseline and 12 weeks post-implementation. A paired-samples t-test was conducted, and the results showed a clinically and statistically significant decline in HbA1c levels from baseline to post-implementation (t(32) = 4.15, p = .001). Based on these findings, implementing a DSMES program in accordance with Standard 5 of the ADA's guidelines may have a positive impact on this patient population. Recommendations include sustaining the project and disseminating the results.
