Polycystic ovary syndrome (PCOS) is the most common female endocrine disorder and is associated with metabolic and psychological presentations. A critical need exists to identify lifestyle approaches that attend to the many presentations of PCOS. The purpose of this three-study dissertation was to examine the need for and acceptability of a lifestyle approach aimed at improving biopsychosocial well-being in individuals with PCOS. Specifically, data were collected using qualitative, quantitative, and mixed methods approaches. Chapter 2 reviewed the existing lifestyle literature aimed at reducing cardiometabolic risk in individuals with PCOS. The review found most studies focused on weight loss, yet studies that modified the diet without an energy deficit saw improvements in cardiometabolic risk. Thus, more sustainable lifestyle interventions that meet the needs of individuals with PCOS across body weights are needed. Chapter 3 explored how PCOS affects food decision making in individuals utilizing the Listening Guide Method of Psychological Inquiry. Results revealed an immense struggle. PCOS posed a challenge in individuals' ability to express wants around food, as they faced external assumptions and pressure to feel they are doing enough to manage their condition. Chapter 4 surveyed levels of intuitive eating, body appreciation, health-related quality of life, psychological well-being, social support, self-silencing, and diet quality in individuals with PCOS. Results demonstrated higher intuitive eaters had more favorable scores of biological, psychological, and social aspects related to PCOS than lower intuitive eaters. Chapter 5 described the preferred intervention characteristics of a weight-neutral approach and the acceptability of intuitive eating in individuals with PCOS. Findings revealed participants were interested in a lifestyle program, consisting of a mix of individual and group sessions delivered in-person and remotely. A registered dietitian was the preferred provider to administer the intervention and many individuals preferred an interdisciplinary team. Participants expressed interest in intuitive eating, with some concerns over letting go of food rules and navigating cravings. A need for an individualized approach with practical guidance was identified. Together, the results of this dissertation highlight the need for a biopsychosocial approach to PCOS and additional research to clarify the possible benefits of intuitive eating for PCOS.
