This qualitative phenomenological study explores the lived experiences of emergency department (ED) healthcare personnel using standardized electronic medical record (EMR) documentation protocols, with a focus on how education and training shape EMR efficiency and patient safety. Despite widespread EMR adoption, human-factor barriers, rigid protocols, and training gaps persist, particularly in high-acuity ED settings. Semi structured interviews with eleven frontline clinicians revealed six core themes: EMRs are indispensable yet often burdensome due to poor usability; standardized protocols promote data integrity but can conflict with urgent workflows; and initial training builds competence but lacks sufficient ongoing reinforcement. Participants emphasized the need for scenario-based refresher training, adaptive, role-specific templates, and integrated learning tools to reduce documentation burden and enhance patient care. The findings underscore that successful EMR implementation is not solely a technical challenge but also a human-centered endeavor requiring continuous, context-driven education and codesigned workflows. By aligning the technology acceptance model (TAM) and socio-technical systems (STS) theory, this study highlights how user-centered design and sustained training can reduce errors, streamline workflows, and optimize patient outcomes in emergency care. The results offer actionable guidance for healthcare leaders, informatics teams, and policymakers committed to bridging the gap between EMR policy and frontline practice.
