This is a report on implementing a telehealth diabetes self-management education program to improve glycemic control among African American patients with Type 2 Diabetes Mellitus. Due to the COVID-19 pandemic, telehealth is especially relevant due to the need to maintain physical isolation to avoid infection. The health belief model (HBM) principles were used to develop the intervention. This project utilized a pre-test/post-test non-experimental design where a group of participants was educated about behavioral approaches to manage their T2DM. The self- management diabetes education intervention was conducted over 12 weeks with one weekly session. Thirty-two participants recruited at a primary care practice completed the program. In Week 1 the mean BG level for all participants was M = 208.8, SD = 30.2, but in Week 12 BG values decreased to M = 153.1 SD = 6.19 (p = .01). The Revised Diabetes Knowledge Questionnaire (DKT2) administered before and after the intervention was used to measure a change in participants' knowledge about T2DM. This analysis indicated that after the program, the number of questions on DKT2 answered correctly increased regardless of the statistic used to measure it. Studies indicate that the intervention improved glycemic control and knowledge about T2DM.
