Breast cancer (BC) survivors may face a higher risk of developing second primary cancers compared to individuals without a BC diagnosis due to shared risk factors and potential carcinogenic effects of cancer therapy. Lung cancer (LC) is the most common second primary cancer among BC survivors. However, the degree to which a prior BC diagnosis increases the risk of subsequent LC remains uncertain, as previous studies have reported mixed findings. In our first analysis, utilizing clinical data from the Prostate, Lung, Colorectal, and Ovarian (PLCO) Cancer Screening Trial, we found BC diagnosis was an independent risk factor for developing a second primary LC after adjustment for established risk factors. Similarly, the improved longevity of prostate cancer (PC) survivors has heightened the clinical importance of understanding and addressing secondary health risks, such as the development of LC. Nonetheless, the risk of LC in PC survivors has been underestimated and remains under-studied. Using the PLCO dataset, we focused specifically on PC patients and assessed whether a PC diagnosis was associated with an increased risk of subsequent LC, accounting for tobacco use. Our analysis did not find evidence to support the hypothesis that PC independently raises the risk of developing second primary LC. As life expectancy becomes more integrated into evidence-based screening and treatment guidelines, it plays a critical role in supporting patient-centered care and clinical decision-making. However, life expectancy estimates from standard life tables do not account for cancer history, comorbidities, or other important predictors for premature mortality. Given that BC survivors have a modestly increased risk of developing second primary LC and may derive benefits from LC screening interventions, we explored data from the National Health Interview Survey to estimate life expectancy, incorporating smoking patterns and comorbidities. We found that life expectancies varied substantially from standard U.S. life tables when adjusted for individual-level smoking behavior and comorbidities. These customized estimates can complement existing risk prediction tools and inform personalized decisions about the benefits and timing of LC screening in this potentially high-risk group.
