The purpose of the qualitative descriptive study is to explore how faith-based leaders perceive African-American congregants' utilization of the family as a system, the environments encompassing them as family, the interrelationships between the family and the healthcare environment, and their role of communicating knowledge to make informed decisions about end-of-life care options in Georgia. The problem addressed is the lack of understanding of the faith-based leaders' influence on African American end-of-life care decision-making. The theoretical foundation is based on the Purnell Model for Cultural Competence and the Family Ecosystem Model. Four research questions examined: (1) How do faith-based leaders perceive African-American congregants' utilization of the family as a system to make informed decisions about end-of-life care options? (2) How do faith-based leaders perceive African-American congregants' utilization of the environments encompassing them as a family to make informed decisions about end-oflife care options? (3) How do faith-based leaders perceive African-American congregants' utilization of the interrelationships between the family and the healthcare environment to make informed decisions about end-of-life care options? (4) How do faith-based leaders perceive African-American congregants' utilization of their role of communicating knowledge to make informed decisions about end-of-life care options? A qualitative design with convenience and snowball sampling was used to recruit participants, leading to 12 individual interviews and two focus groups. Data were analyzed using Braun and Clarke's six steps of thematic analysis. The findings reveal the significant roles faith-based leaders play in the decision-making processes of African American congregants regarding end-of-life care, emphasizing the intricate interplay of cultural values, family dynamics, and healthcare settings. The study concludes that faith-based leaders are pivotal in guiding congregants through culturally sensitive and informed end-of-life care decisions.
