Tumorigenesis in the colon is a multi-step process. Colon polyps arise from two pathways, the classic adenoma to carcinoma sequence or the serrated pathway. Each pathway contains distinct molecular characteristics, which leads to variation between lesions or intertumoral heterogeneity. Though the genetic alterations for each pathway are well known, it is unclear which polyps will remain polyps, and which polyps will progress to invasive cancers. Intertumoral heterogeneity is also prevalent in colorectal cancers. Based on similar molecular characteristics, colorectal cancers can be subtyped to better understand treatment responses and patient outcomes. The current methodology to subtype colorectal cancers greatly limits its use in clinical practice, therefore, alternative methods have been developed and now require validation. Not all colorectal cancers within the same molecular subtype respond to the same therapies. This may be due to intra-tumoral heterogeneity or different clonal populations within a tumor. The development of new mouse models that recapitulate tumor heterogeneity would aid in understanding how heterogeneity impacts tumor development and response to therapies. This dissertation will explore how inter- and intra-tumoral heterogeneity at various stages in the continuum of normal colon to polyp to cancer impacts lesion growth and therapeutic response.
