Background: Young female athletes are at an increased risk of menstrual dysfunction (MD), because of high intensity training and low energy intake. Additionally, these factors have been associated with an increased risk of musculoskeletal injury including stress fractures. Physical Therapists who provide musculoskeletal interventions to female athletes may not have the knowledge of the relative energy deficiency in females and its associated risk factors. This meta-analysis aimed to quantify the effect of MD on whole body (WB) and lumbar bone mineral density (BMD) as well as body mass index (BMI) in the population of high-impact female athletes ages 12-25 compared to eumenorrheic female athletes. Methods: An electronic database and journal search was conducted from December 2023 until August 2024. Six studies met the inclusion criteria and were further assessed for study quality by a single reviewer using the Appraisal Tool for Cross-sectional Studies. Results: Athletes with MD had statistically significant lower WB BMD (effect size: 0.83; 95% CI: 0.58-1.09; I2: 0%) and lumbar BMD (effect size: 0.79; 95% CI: 0.58-0.99; I2: 0%) as well as BMI (effect size: 0.91; 95% CI: 0.61-1.30; I2: 36.86%) compared to the eumenorrheic athletes. Conclusion: High impact female athletes with MD had significantly lower WB and lumbar BMD suggesting reduced benefits of mechanical loading. Lower BMI may contribute to the BMD findings, so the results cannot be entirely attributed to the presence of menstrual dysfunction. However, this meta-analysis highlights the importance for physical therapists to screen female athletes for relative energy deficiency in sports (RED-S) to mitigate the risk of musculoskeletal injuries.
