Background. Intravenous drug users (IVDUs) are the major HIV-1 infected group in China. More young males continue to be recruited to drug use. Objectives. To identify the risk factors for drug use, intravenous drug use (IVDU) and sharing paraphernalia, and to assess HIV/AIDS knowledge level. Methods. Cross-sectional and non-concurrent cohort designs were used. Demographic, behavioral, and drug using related information was collected by an interviewer administrated anonymous questionnaire from males aged 18 to 29 years in 82 villages with a high prevalence of heroin users in Longchuan. Drug use, IVDU, and sharing equipment were used as outcome variables. January 1, 1991 to August 1, 1994 was defined as the study period. Univariate and multivariate analytic strategies were used. Results. A total of 1,548 subjects were interviewed. The cumulative incidence were 17.2% (95% C.I.:15.2-19.4) for drug use and 40.0% (95% C.I.:35.2-45.0) for IVDU among drug users. The annual incidence of IVDU increased yearly from 1991 to 1994. Risk factors for drug use included being Jingpo (OR = 1.8, 95% C.I.:1.2-2.5), being divorced/widowed/separated (OR = 8.9, 95% C.I.:1.8-43.3), cigarette smoking (OR = 2.4, 95% C.I.:1.6-3.8), having had pre-/extra-marital sex (OR = 1.5, 95% C.I.:1.1-2.2), having received encouragement by friends/others to try drugs (OR = 8.8, 95% C.I.:6.1-12.9), and having a family member who used drugs in 1991 (OR = 1.5, 95% C.I.:1.0-2.3). More than six years of education was a protective factor for drug use (OR = 0.6, 95% C.I.:0.4-0.98). Risk factors for IVDU included having had pre-/extra-marital sex (OR = 1.5, 95% C.I.:1.01-2.3) and having a family member who used drugs in 1991 (OR = 1.8, 95% C.I.:1.1-2.9). Being buddhist was a protective factor for IVDU (OR = 0.4, 95% C.I.:0.2-0.9). Risk factors for sharing equipment included being Jingpo (OR = 15.8, 95% C.I.:1.9-133.0) and first used drugs with friends/others (OR = 5.8, 95% C.I.:1.1-30.9). Not being a farmer was a protective factor for sharing equipment (OR = 0.3, 95% C.I.:0.1-0.9). The overall knowledge of HIV/AIDS was low. Conclusions. Personal cigarette smoking and pre/extra-marital sexual behaviors, family members' drug using behavior and peers' encouragements strongly influence an individual in taking drugs. Community-based interventions are urgently required. Intervention efforts need to focus on young males before age 15 years and continue intensively through at least age 26 years.
