Recovering quadriceps function following anterior cruciate ligament (ACL) reconstruction is central to clinical practice guidelines but the relationship with successful outcomes remains equivocal. Isokinetic testing is a frequently reported method to measure quadriceps strength; however, test interpretation is typically limited to a single point of the torque-time curve. Although rarely investigated, quantifying the smoothness of the curve could reveal novel insights into neuromuscular function and open avenues to improve post-operative outcomes. The medical records of adolescents, who underwent ACL reconstruction at a single pediatric hospital and completed a return-to-play (RTP) battery of tests 6 months post-operatively, were reviewed retrospectively. The objectives were to describe isokinetic knee extension torque steadiness in both the time and frequency domains and to explore its relationships with potential predictive factors and RTP testing outcomes across multiple levels of the International Classification of Functioning, Disability, and Health (ICF) framework.Poorer knee extension torque steadiness, quantified as a coefficient of variation (CV) and a peak power frequency (PPF), was observed in the involved limb compared to the uninvolved limb. Both involved limb CV and PPF were weakly to moderately associated with RTP testing outcomes, although CV was the only measure significantly correlating with outcomes across multiple levels of the ICF framework. The cumulative findings of the project identify isokinetic knee extension torque steadiness measures as promising novel approaches complementing traditional strength testing methods to provide a more complete picture of quadriceps function and improve the care of young athletes following ACL reconstruction.
