Age-related vascular degeneration is a major factor in several cerebrovascular and neurodegenerative diseases, including stroke, Alzheimer's disease (AD), and AD-related dementia (ADRD). The early and noninvasive identification of these age-related vascular changes, prior to the onset of symptoms, is crucial for improving our understanding of disease mechanisms and developing effective preventive treatments. This dissertation aims to utilize advanced quantitative magnetic resonance imaging and MR angiography to evaluate changes in vascular morphology and blood flow of both large and small arteries, as well as the capillary glomus in the choroid plexus, and to assess their impact on brain health in the context of normal aging. Firstly, we employed time-of-flight (TOF) and phase-contrast MRI (PC-MRI) to quantitatively evaluate the tortuosity of large brain-feeding arteries. Our findings revealed associations between vascular tortuosity, blood flow decline and higher volume of white matter hyperintensities in the context of aging. Secondly, using high-resolution Ferumoxytol-enhanced gradient echo (GRE) imaging, we achieved a pioneering visualization of the corkscrew appearance of cerebral small arteries at a microscopic scale (50-200 μm) by inducing blooming effects and susceptibility shifts on 7T MRI, showing an age-related increase in their quantity in vivo. The count and severity of these tortuous cerebral small arteries are closely linked to body mass index (BMI) and enlarged perivascular spaces (PVSs). Thirdly, we quantitatively assessed the age-related decrease in vascular density and perfusion in the ChP based on 3T and 7T MRI, with and without contrast, in datasets of normal aging individuals. In summary, as aging is the predominant demographic risk factor for dementia, aging-related vascular degeneration may contribute significantly. Our findings suggest that MRA with advanced post-processing for tortuosity and quantitative MRI provide objective and effective assessments of vascular aging, even in healthy individuals in their fifties. These methods also highlight potential implications for AD/ADRD.
