Clinicians may err when managing complex patients. Clinical practice guidelines (CPGs) contain evidence-based recommendations intended to improve patient care. Specifying CPGs formally as computer-interpretable guidelines (CIG) enables their execution over a patient's Electronic Health Record (EHR) to deliver patient-specific recommendations, supporting clinicians. However, integration of CIG concepts with EHR data is difficult due to the conceptual gap between CIGs and EHRs. CIGs are developed in a generic way that does not make assumptions about how EHR data is organized and often refer to more abstract terms than the fields of the EHR. To bridge this gap, we need to define a mapping of abstract medical concepts from CIGs to EHRs, and automatically translate the mappings into queries that return the EHR data in the form of the needed CIG abstractions. This approach has been named Knowledge Data Ontology Mapping (KDOM) and has been implemented before but with several key functionalities which were missing in terms of both methods and implementation. We have developed a new KDOM method that leverages standard patient information HL7 Fast Healthcare Interoperability Resources (FHIR) standard and defines an ontology of mapping classes that could completely support the mappings of CIG abstractions. The new KDOM supports temporal mappings, completing missing values while allowing some interpolation of missing data points in time, better supports hierarchical terminology mapping as well as listens to triggering events and responds to them using RESTful API. A special algorithm was developed, as part of KDOM, and implemented to automatically create EHR queries based on ontology mapping instances. KDOM has been tested and utilized with interfacing DSS components over the CAPABLE (CAncer Patients Better Life Experience) Environment.
