Globally, millions of infants and toddlers are in institutional care. In Hong Kong, this care supports children lacking adequate family support due to neglect, abuse, economic hardship, mental health issues, or substance use/abuse. Prenatally substance-exposed children often struggle with physiological regulation, social and emotional control, and sleep. In 2019, the author of this study launched the Infant Cuddling Program (ICP) at a local institutional care center in Hong Kong. The ICP is an 8-week cycle intervention that uses trained volunteers, known as cuddlers, to provide a supportive touch to nurture children's self-regulation and emotional and social development. This study considered the importance of early relational intervention services for children awaiting family reunification or adoption, a process that can often take years. Using a quasi-experimental, within-subject, repeated measures design, ICP program data from 44 confirmed or suspected prenatally substance-exposed infants under 6 months of age were analyzed to evaluate the ICP's effectiveness on crying, sleep, and interpersonal interactions. Using multivariate analysis of variance (MANOVA) and repeated measures analysis of variance (ANOVA), results indicated statistically significant improved sleep and reduced crying, demonstrating the program's beneficial impact. Consistent with existing literature on the benefits of physical touch, study findings highlight elements of the ICP program as an early intervention strategy to support developmental outcomes. However, no statistically significant findings were found related to changes in infant interaction with the cuddlers. Future studies may evaluate the impact of a longer ICP cycle or longitudinal impact of ICP on infant-cuddler interactions. By evaluating the ICP program effectiveness, this study sought to provide insights into interventions that promote early intervention services. The current study findings suggest that the ICP has the potential to enhance care for at-risk institutionalized infants in Hong Kong, thereby improving their current well-being and enhanced long-term relational outcomes for these vulnerable children.
