Human brain cells coordinate essential functions like cognition, behavior, and motor control. In neurodegenerative diseases like Alzheimer's, these cells undergo reactive changes and death, often accumulating misfolded proteins called proteopathies. Studying this single-cell landscape has been difficult due to limited access to diverse brain samples and inadequate methods for capturing and analyzing single-cell differences. Using multiplex ion beam imaging (MIBI), we developed a platform to map single-cell phenotypes and their spatial organization in healthy and Alzheimer's disease brain tissue. This revealed immune-activated microglia surrounding proteopathic structures and neurons resistant to damage, particularly in advanced disease stages. We further analyzed the spectrum of microglial phenotypes using trajectory mapping, identifying continuous expression profiles across brain regions in both healthy and Alzheimer's contexts. Differences in immune activation profiles between these groups suggest altered microglial functions in neurodegeneration. Additionally, a novel deep learning-based method was developed to analyze tissue textures, revealing broader proteomic changes independent of cellular structures. This work integrates detailed proteomic data with tissue-level organization, offering new insights into human brain phenotyping and the underlying biology of neurodegenerative diseases.
