Although substance use among adults over the age of 65 has escalated to epidemic levels, substance use disorders among older adults have continued to be overlooked, underdiagnosed, and undertreated by healthcare providers, including counseling professionals. The problem addressed by this study was counselor-level barriers in diagnosing and treating substance use disorder in older adults. Existing research on barriers to effective clinical work with older adults has primarily focused on counseling students and beginning practitioners, which may not reflect the realities of seasoned counseling professionals. The purpose of this qualitative descriptive phenomenological study was to explore seasoned counseling professionals' lived experiences of the diagnostic and treatment process of older adults experiencing substance use disorders, including their perceptions of their individual barriers to appropriately recognizing and meeting older adults' substance use disorder treatment needs. Self-efficacy theory served as the framework for this study of 20 purposively sampled seasoned counseling professionals licensed in Florida. Data were collected through semi-structured online interviews using open-ended questions and analyzed using Amedeo Giorgi's five-step phenomenological data analysis method, generating 25 constituents to answer the six research questions of this study. While findings of this study converged with prior research regarding importance of rapport, need for collaboration with medical providers, and early career challenges with asking about substance use, results diverged from prior research findings regarding expectations of use, consideration of misuse, universal screening, pathways to substance use disclosure, ageism, and interest in and enjoyment of working with this population. Clinical experience was key to developing competence and confidence in diagnosing and treating substance use disorders in older adults. These findings imply clinical experience can profoundly shape behavior, attitude, attention, and motivation of counseling professionals in diagnosing and treating older adults experiencing substance use disorders. Implications for practice are to increase counselor exposure to this population during early career. Future research might explore how recognizing and meeting the needs of older adults with substance use issues is impacted by counselor developmental process.
