This qualitative phenomenological-descriptive study explores the multiple lived experiences of mass shooting survivors through the lens of psychotherapists who have provided treatment subsequent to the trauma. The study investigates the unique traumatology and responsiveness to subcortically targeted treatments aligned with Adaptive Information Processing theory within a diverse sampling of mass shooting survivors, to include both children and adults. Through structured interviews and drawing on participants' review of case notes, this study examines symptoms presented, themes identified for target processing, areas of impairment and of strength to include impacts on interpersonal relationships and spirituality, treatment outcomes, and vicarious impacts upon psychotherapists. Comparison is made of symptom presentation differentials between survivors of mass shootings wherein the fellow survivors and victims are known and those wherein most victims are unknown to the survivor. Participants are drawn from multiple states across the USA and represent response to both crisis and long-term treatment using recent events, group, and standard protocols. While Acute Stress or PTSD were universal diagnoses amongst the survivors treated, symptom patterns coalesced into predictable clusters within the diagnostic criteria, suggesting a proposed Massacre Survivor Syndrome. Survivor patients responded with universal positive outcomes to subcortical therapies to include EMDR and Brainspotting. Delays in treatment access were found to correlate with a higher number of sessions required and more modest symptom reduction. Therapists providing treatment to mass shooting survivors may be at risk for vicarious trauma symptoms and should enact self-care practices that include confidential peer debriefing and /or personal psychotherapy.
