Clinical Trial Networks (CTNs) are essential for advancing healthcare by conducting multi-site clinical trials that enhance evidence-based care and expedite the integration of findings into clinical practice. However, existing evaluation metrics are fragmented, often focusing on isolated domains such as clinical outcomes, economic impact, or operational efficiency, which hinders a comprehensive understanding of CTNs' effectiveness. The study aimed to develop a conceptual framework for evaluating CTNs effectiveness, focusing on the Cardiothoracic Surgical Trials Network (CTSN). The central research question was: "What factors contribute to the effectiveness of a clinical trial network?" We hypothesized that a comprehensive evaluation integrating clinical, economic, and social metrics would provide a more accurate understanding of CTNs impact than single-dimensional metrics. An empirical mixed-methods study design was employed, combining quantitative and qualitative techniques. The specific aims were: (1) to identify existing evaluation frameworks through a comprehensive scoping review; (2) to systematically analyze NIH-funded CTNs to understand their structures and operations; (3) to perform an in-depth organizational analysis of the Cardiothoracic Surgical Trials Network (CTSN); and (4) to develop a conceptual framework - the Research Alignment Model - that integrates clinical, economic, and social metrics.Findings revealed that current evaluation approaches are insufficient to capture the multidisciplinary nature of CTNs. The Research Alignment Model addresses this gap by providing a tailored framework comprising key components essential to CTNs operations: strategic partnerships, core activities, key resources, stakeholder engagement, value propositions, funding and allocation, dissemination channels, and impact and outcomes. The model offers specific indicators and metrics for each component, enabling comprehensive assessment of CTNs effectiveness. The Research Alignment Model presents a structured approach to evaluating and enhancing CTNs effectiveness. By adopting this framework, CTNs can better align operations with strategic goals, optimize resources, and ultimately improve patient outcomes and advance clinical research.
