Patients with end-stage renal disease who receive hemodialysis can face significant obstacles in managing their nutrition and hydration intake. At the project site, no nutrition guidelines were in place, so an evidence-based solution was sought. The purpose of this quality improvement project was to determine if the implementation of the Kidney Disease Outcome Quality Initiative (KDOQI) Guideline for Nutrition in Chronic Kidney Disease for intradialytic sodium and fluid overload would impact interdialytic weight gain (IDWG) among patients with end-stage kidney disease (ESKD). The project was piloted over 12 weeks at an urban Florida hemodialysis facility. Virginia Henderson's nursing needs theory, and James Prochaska and Carlo DiClemente's transtheoretical model of health behavior change provided the project's theoretical underpinnings. A total of 52 patients were included in the project. Weight data to compute IDWG were collected at baseline and 12 weeks post-implementation from the electronic health record. A paired t-test showed a statistically significant reduction in IDWG from baseline (M = 2.57, SD = 1.35) to post-implementation (M = 2.06, SD = 1.05), t (51) = 2.14, p = .037. Clinical significance was supported by the 19.8% reduction in mean IDWG. Based on these findings, providing nutritional guidance using the KDOQI guideline may positively impact interdialytic weight gain for patients with ESKD. Recommendations include sustaining the project at the site and disseminating the results.{A0}
