Purpose: The aim of this study was to assess the nutrition knowledge of pregnant people in the U.S., evaluate their diet quality, and explore the potential association between food security, diet quality, and nutrition knowledge. We hypothesized that pregnant individuals with higher nutrition knowledge will be more likely to have a better diet quality. And participants with food insecurity will be more likely to have poorer diet quality regardless of the nutrition knowledge level.Methods: 211 participants were recruited through the Cloud Research survey platform. All study instruments were administered online through REDCap after participants consented. Participants completed the National Health and Nutrition Examination Survey Food Frequency Questionnaire (NHANES FFQ), Pregnancy Nutrition Knowledge Questionnaire (PNKQ), and U.S. Department of Agriculture 6-item Food Security Survey with Nutrition Security questions.Results: The average nutrition knowledge score among participants was 41 points out of 74 points (55%). There was a main effect of pregnancy nutrition knowledge on the intake of fruits, protein, dairy, foods to eat, refined grains, added sugar, alcohol, sodium, saturated fat, and foods to avoid. Food security status influenced intake across all food categories, where individuals with food insecurity consumed higher amounts compared to individuals with food security. Participants with food insecurity and low pregnancy nutrition knowledge had higher alcohol consumption.Conclusion: Overall, nutrition knowledge was poor among participants. A significant observation from this study was the higher alcohol consumption among the lower pregnancy nutrition knowledge and food insecure group, highlighting the necessity to address alcohol consumption during pregnancy. The findings suggest that nutrition education targeting lower socioeconomic groups could help alleviate the impact of socioeconomic status and food insecurity on diet quality. This is particularly crucial for educating about foods to avoid during pregnancy and tackling the issue of alcohol intake.
