This secondary analysis of two pilot randomized controlled trial on older adults with chronic pain (MORPH & MORPH II) aimed to investigate the relationship between baseline daily movement patterns and changes in pain and Health-Related Quality of Life (HRQoL). METHODS: Participants (N=68; 69.54 ± 6.74) wore an ActivPAL4 triaxial activity monitor continuously, completed the 36-item Short Form survey, and PROMIS pain intensity and interference scales at week 0 and week 12. A functional principal components analysis (fPCA) was performed to identify profiles of daily movement patterns. RESULTS: Component 1 (52.8% variance explained) scores differentiated subjects engaging in more vs less activity across the day. Component 2 (16.1%) scores captured those who took more vs less time to begin moving in the day. Higher component 1 scores were associated with more positive changes in HRQoL subdomains (physical function, emotional role limitation, energy/vitality, social functioning, pain, and general health), and reductions in pain intensity. Higher component 2 scores were associated with worsening scores in unique HRQoL subdomains (physical function, physical role limitation, and general health) and worsening pain interference. CONCLUSION: Future intervention development may benefit from targeting overall movement while providing additional support for those who struggle to move early in the day due to pain.
