Thesis (Psy.D.)--The Chicago School of Professional Psychology, 2021.
A comprehensive literature review was conducted to obtain psychological and neuropsychological testing data and distinctive histories or patterns associated with individuals diagnosed with conversion disorder, factitious disorder, and malingering that may help distinguish among their presentations and underlying motivations. Computer databases were utilized to locate articles, book chapters, and relevant psychological and neuropsychological measures that focus on these three presentations. The common histories, potential neuroimaging correlates, and potential testing data for each diagnosis were identified to create a best practice guide to help clinicians decide what steps to take when confronted with this differential diagnosis. It was predicted that, although there are more theoretical similarities between factitious disorder and conversion disorder, data from psychological tests would show more similarities between factitious disorder and malingering. The review revealed that there is substantial overlap that prevents these presentations from being fully distinct from one another, and because the field of psychology is moving toward a dimensional model of diagnosis to more accurately capture symptom presentations (Maj, 2018), this dissertation argues that these diagnoses are best described using a dimensional approach to account for the nuances that differentiate between various presentations of symptom production diagnoses. The best practices guide is offered as a scaffold to be built upon by further research comparing these three presentations.
