The increasing prevalence of Type 2 Diabetes (T2D) has led to extensive research focusing on its impact on the left ventricle (LV) of the heart, while the role of the right ventricle (RV) remains understudied. However, the RV is linked to heart failure in individuals with T2D, and its dysfunction may manifest before clinical symptoms become apparent. Our study explores RV function in individuals with T2D and obesity using cardiac magnetic resonance (CMR) imaging, with sex differences. Pre-exercise analysis reveals that T2D and obesity are associated with lower RV volumes compared to a reference control group, with T2D showing more significant impairments. However, post-exercise analysis demonstrates improvements in RV function, particularly in the T2D group, indicating exercise's positive impact on RV function. Differences in ventricular function between males and females are also observed, with males demonstrating more improvement in right ventricular function after exercise. However, obese individuals exhibited minimal improvement in their RV function after exercise. These findings underscore the importance of exercise as a potential therapeutic intervention for improving right ventricular function in individuals with T2D, with potential sex-specific responses. All RV parameters strongly correlated with their corresponding LV parameters, specifically before exercise.
