Background: Osteoarthritis (OA) is a degenerative disease where the articular cartilage within a joint wears away causing pain, movement limitations and bone deformities. For knee OA alone, the US spends over 27 billion in healthcare. While exercise is the key to recovery for patients coming into physical therapy, evidence mentions other aspects within the therapist tool kit that may assist patients with return to function and pain reduction. Manual therapy is a common intervention for knee OA patients that involves hands on techniques from the physical therapist ranging from effleurage, soft tissue mobilization to joint mobilizations for the purpose of pain relief, increasing ROM and more. Transcutaneous electrical nerve stimulation (TENS) is a form of electrotherapy where an electrical current is applied that stimulates nerves ranging from sensory to motor fibers for the purpose of pain relief, strengthening and more depending on the parameters. Objective: The primary goal of this meta-analysis was to determine whether TENS or manual therapy when either are paired with exercise will improve function and pain within older adults with knee OA. Methods: The literature search started December of 2021 and ended on September 2022 looking through OneSearch, PubMed, Cochrane and CINAHL. Results: Primary analysis found manual therapy to be superior to TENS for improving function and pain. However, statistical analysis for pain was to be accepted with caution. Conclusion: Results from this meta-analysis suggested that manual therapy with exercise may be more effective than TENS for improving function within older adults with knee OA. Further research was required for confirming whether manual therapy was superior for pain reduction.
