Since the 1980s, Haiti has experienced persistent political instability, natural disasters, food insecurity, and gang violence. The country has low health indicators and natural resource management challenges, creating a unique planetary health scenario where human and ecosystem health are inextricably linked. To secure planetary health benefits in Haiti, there is a need to scale up sustainable natural resource management (NRM) initiatives and programs. Consequently, this research took on several objectives to provide recommendations for NRM implementation in Haiti. I adopt the Planetary Health, States of Fragility, and Scaling Innovations frameworks and utilize a case study methodology to investigate how NRM programs in Haiti are designed, implemented, scaled, and sustained in the face of fragility. I also document the effectiveness of implementing a planetary health approach to provide human and ecosystem health outcomes in Haiti. The results of the research deliver several key findings. After examining the implementation and effectiveness of three community-based planetary health approaches, I find that integrating public health, clinical care, and natural resource management to address rural health and livelihoods is an effective programmatic approach. Human nutrition served as an entry point for NRM in rural communities; NRM then provided ecosystem health benefits. An analysis of how contextual factors influence the establishment, implementation, and scaling of NRM programs showed that relational factors such as social capital, networks, and collaborations are crucial to program success. Financial resources and adaptation to local contexts also highly influenced NRM program success. Lastly, this research identified various adaptive strategies actors utilized to cope with Haiti's fragility and maintain program operations. The results of this third study show that devolving ownership to locals and grassroots organizations was associated with continued implementation throughout several periods of shock and instability. Combined, these findings from the three studies produce several recommendations and action items for actors engaged in NRM in Haiti and similar contexts. I argue that policy and international development transformations are necessary to secure the planetary health benefits of NRM practices in fragile settings.

Depuis les annees 1980, Haiti est confronte a une instabilite politique persistante, a des catastrophes naturelles, a l'insecurite alimentaire et a la violence des gangs. Le pays presente de faibles indicateurs de sante et des defis de gestion des ressources naturelles, creant un scenario de sante planetaire unique ou la sante humaine et celle des ecosystemes sont inextricablement liees. Pour garantir les benefices pour la sante planetaire en Haiti, il est necessaire d'intensifier les initiatives et les programmes de gestion durable des ressources naturelles (GRN). Par consequent, cette recherche a poursuivi plusieurs objectifs pour fournir des recommandations pour la mise en oeuvre de la GRN en Haiti. J'adopte les cadres de sante planetaire, d'etats de fragilite et de mise a l'echelle des innovations et j'utilise une methodologie d'etude de cas pour etudier comment les programmes de GRN en Haiti sont concus, mis en oeuvre, mis a l'echelle et soutenus face a la fragilite. Je documente egalement l'efficacite de la mise en oeuvre d'une approche de sante planetaire pour obtenir des resultats en matiere de sante humaine et ecosystemique en Haiti. Les resultats de la recherche fournissent plusieurs conclusions cles. Apres avoir examine la mise en oeuvre et l'efficacite de trois approches communautaires de sante planetaire, je trouve que l'integration de la sante publique, des soins cliniques et de la gestion des ressources naturelles pour aborder la sante et les moyens de subsistance en milieu rural constitue une approche programmatique efficace. La nutrition humaine a servi de point d'entree pour la GRN dans les communautes rurales ; La GRN a ensuite apporte des avantages a la sante des ecosystemes. Une analyse de la maniere dont les facteurs contextuels influencent l'etablissement, la mise en oeuvre et la mise a l'echelle des programmes de GRN a montre que les facteurs relationnels tels que le capital social, les reseaux et les collaborations sont essentiels au succes des programmes. Les ressources financieres et l'adaptation aux contextes locaux ont egalement fortement influence le succes du programme de GRN. Enfin, cette recherche a identifie diverses strategies d'adaptation utilisees par les acteurs pour faire face a la fragilite d'Haiti et maintenir les operations du programme. Les resultats de cette troisieme etude montrent que le transfert de propriete aux organisations locales et de base a ete associe a une mise en oeuvre continue tout au long de plusieurs periodes de choc et d'instabilite. Combines, ces resultats des trois etudes produisent plusieurs recommandations et mesures a prendre pour les acteurs engages dans la GRN en Haiti et dans des contextes similaires. Je soutiens que des transformations politiques et de developpement international sont necessaires pour garantir les bienfaits des pratiques de GRN sur la sante planetaire dans les contextes fragiles.
