The problem addressed in this study was understanding compliance outsourcing's capability to promote a culture where employees act with integrity and accountability. The study's findings impact skilled nursing care geriatric resident's quality of life. The study findings also help healthcare researchers and healthcare leaders identify, understand, and develop the best method to reach informed strategic resource decisions that buffer healthcare environmental risks. The healthcare organizational setting operates in a highly regulated business sector with evolving changes due to ethical business misconduct. The healthcare research site was a skilled healthcare organization chosen to capture the complexity and peculiarities of the specific perspectives of compliance outsourcing in healthcare. The study participants were a mix of 12 frontline to senior leader employees who experienced working with an outsourced compliance service provider. The purpose of this qualitative Delphi study modified to grounded theory design was to understand the value of outsourcing compliance capabilities to achieve the organizational spread and adoption of the importance of integrity, accountability, and how ethical non-compliance impacts the performance of a healthcare organization was key to reaching company goals. The negative effect of non-compliance causes harm to the quality of life of the elderly population, such as fear of retaliation, sleep deprivation, loss of appetite, depression, and even death. The modified Environment, Strategy, Core Competencies, Organization, Leadership framework (ESCO) was meaningful for evaluating and identifying organizational misalignment that conceptually correlated to business outsourcing to use its capabilities to achieve value and the organization's strategic intent. The study findings demonstrate organizational underperformance linked to repeat non-compliance secondary to a misalignment between the outsourced compliance strategy in a business sector characterized by pervasive non-compliance. The study findings disclosed how internal factors in the healthcare environment, such as compliance governance, strategic leadership decisions, combined with the external factor of third-party capability execution risks, can lead to organization misalignment. The results of this study provide a starting place for further research using archival data to point out and measure the state and federal regulatory non-compliance of a healthcare organization that employs outsourced compliance as a strategy to mitigate pervasive non-compliance. The implications of the study findings informed the research questions by pointing out operational risks of compliance business outsourcing in healthcare and the issues that played a part in an environment- performance imbalance. The study findings offer new insight and add to existing knowledge on organizational theory, advanced strategic planning, and the concept of business outsourcing.
