Cerebral palsy, or CP, is a common congenital disorder with an incidence of 2-3.6 per 1000 individuals globally. CP causes cognitive and motor deficits, with the majority of individuals presenting with impaired gait. Treadmill training has been used as a task-based intervention geared toward improving the gait cycle. Standard physical therapy, including resistance, balance, and endurance interventions, has been used by many clinicians to improve gait function in individuals with CP. For this meta-analysis, PubMed, CINAHL Plus with Full Text, SPORTDiscus, and Cochrane Central Register of Controlled Trials were used with the following search terms: cerebral palsy, physical therapy, treadmill training, and gait. 11 articles met the inclusion and exclusion criteria for this metaanalysis. All studies included at least one of the intended outcome measures, the 6- minute walk test (6MWT) or 10-meter walk test (10MWT), and included at least one of the intended interventions, treadmill training, or standard PT. The 6MWT, measuring gait endurance, had heterogenous results favoring treadmill training and a small to medium grand effect size of 0.20. The 10MWT, measuring gait speed, had heterogenous results favoring standard PT and a very large grand effect size of 0.94. Treadmill training and standard PT both yielded positive results in all studies. However, treadmill training may be preferred for improvements in gait endurance, while standard PT may be preferred for changes in gait speed.
