Since emerging in Wuhan, China in 2019, SARS-CoV-2, the virus that causes coronavirus disease 2019 (COVID-19), has had a devastating impact on public health and well-being globally (Chan et al. 2020, Zhu et al. 2020, University 2021). Risk for exposure to SARS-CoV-2 and risk for developing severe COVID-19 disease vary by different biologic and sociodemographic factors (Chen et al. 2020, Garg et al. 2020, Holshue et al. 2020, Stokes et al. 2020, Wortham et al. 2020, Van Dyke et al. 2021, Chowkwanyun and Reed 2020, Prevention 2020, Moore et al. 2020, Webb Hooper, Napoles and Perez-Stable 2020, Podewils et al. 2020, Lee et al. 2021). Currently in the United States, the COVID-19 vaccine provides the best protection against COVID-19 and is the most efficient means for combating the pandemic, but vaccine hesitancy remains a major obstacle to reducing the burden of COVID-19. The COVID-19 Community Research Partnership, a large multi-site prospective cohort study in the Southeastern United States, was established to better understand COVID-19 through daily self-reported syndromic surveillance, serially collected COVID-19 serologic surveillance, and electronic health record (EHR) extraction. Later in the study period, this also included understanding factors related to COVID-19 vaccine uptake and COVID-19 vaccine hesitancy. Enrollment occurred from April 2020 through April 2021, with all study surveillance activities ending October 30, 2021. Periodic supplemental surveys were administered to participants that assessed individual and household level characteristics and COVID-19 vaccine uptake and hesitancy. This study used those surveys to achieve the overall research objectives, which includes assessing sociodemographic factors associated with self-reported COVID-19 illness, time to COVID-19 vaccine uptake, and COVID-19 hesitancy through cross-sectional mixed effects multivariable logistic regressions and shared frailty multivariable Cox proportional hazards logistic regressions. Being of older age, residing in an urban county, being college educated, and having a history of tobacco use were all associated with a decreased odds of self-reporting a positive COVID-19 test, while being a health care worker and having increased risk for household level exposure showed an increased odds for self-reporting a positive COVID-19 test. When considering time to first dose of the COVID-19 vaccine from date vaccine eligibility older age, having at least a college degree, having a history of receiving the flu shot, and tobacco use were associated with a shorter time to vaccine uptake, while having increased for household level exposure, and being a healthcare worker were associated with a longer time to vaccine uptake. Additionally, being non-Hispanic Black, being a health care worker, and having increased risk for household level exposure were all associated with increased odds of vaccine hesitancy. The results of this study may help future public health programs target populations that are high-risk for COVID-19 exposure and illness and can assist with future public efforts focused on addressing and overcoming COVID-19 vaccination barriers. Understanding the complex interrelationships of sociodemographic factors that contribute to COVID-19 infection and COVID-19 vaccine uptake and hesitancy is important for more targeted future public health interventions.
