Problem: The incidence of obesity and type 2 diabetes mellitus (T2DM), for which obesity is the leading risk factor, is increasing. Both diseases, especially when comorbid, result in significantly higher all-cause mortality and morbidity. Weight loss and dietary modifications remain one of the most important therapeutic approaches to their management, and research over the past decade has shown promising results in the implication of ketogenic diet. However, all studies evaluating the diet and its impacts lasted less than two years, failing to investigate long-term impacts of the diet past two years. Purpose: This thesis aims to evaluate the efficacy of the ketogenic diet in achieving sustainable weight loss lasting more than two years in obese patients suffering from type 2 diabetes. Research Question: Is the ketogenic diet efficacious in achieving long-term weight loss over the span of five years in overweight and obese patients with type 2 diabetes? Methods: A literature review was conducted using PubMed for articles published between November 2011 and November 2021 using the search phrase: "(low-carbohydrate diet AND low-fat diets) OR ketogenic diet AND (obesity OR overweight) AND weight loss AND diabetes". Studies that were not clinical trials or randomized controlled trials, that did not focus on weight loss, or evaluated populations other than those specified were eliminated. Outcomes: The initial literature search yielded 302 articles. Those were screened for the aforementioned exclusion and inclusion criteria, narrowing the results to 21 articles. These were fully evaluated to select the most relevant, of which ten remained for a final literature review. Benefit: The literature review highlighted the efficacy of the ketogenic diet in achieving weight loss in obese and obese with T2DM patients, and demonstrated the many benefits the diet has outside weight loss on disease burden. However, long-term data surpassing the two year mark observing the lasting impacts of the diet are lacking, and thus further investigation into the ketogenic diet is needed to determine its place, if any, in the standardized treatment of obesity with T2DM.
