Background: Cryotherapy is commonly applied to injuries to promote healing in many clinical settings. Cytotherapeutic modalities are thought to help facilitate healing by using analgesic and anti-inflammatory actions. Purpose: The purpose of this study was to provide an updated systematic review on the use of ice in treating acute soft tissue injuries. Methods: Following PRISMA guidelines, digital databases were searched for randomized control trials evaluating the efficacy of ice on acute soft tissue injures. Eligible studies included human subjects recovering from acute soft tissue injuries, utilizing cryotherapy in any form in isolation or in combination with other therapies, as one of the interventions for comparison of results. Studies were excluded if chronicity of the injury was unspecified or not acute. Two reviewers independently assessed the bias of included trials using the Physiotherapy Evidence Database (PEDro) scale. Results: Eight randomized control trials met the inclusion criteria, with a mean PEDro score of 6.9 out of10. Discussion: Cryotherapy alone may produce analgesic effects in the subacute phase of recovery; however, early exercise may contribute to the positive results as well. No side effects of cryotherapy were reported in any of the included studies. Only six of the eight included studies assessed cryotherapy after acute lateral ankle sprains; therefore, data was limited regarding other acute soft tissue injuries. Obtaining further investigation or studies focusing on other sprains, strains, or muscle contusions would be beneficial for a better understanding of cryotherapy and the relation to acute soft tissue injuries. Conclusion: While cryotherapy can be effective at reducing pain after acute injury, more varied and high-quality research is needed on the application of cryotherapy to acute soft tissue injuries.
