Independent mobility is a key milestone for children in their development, enables self-exploration, and increasing self-esteem, peer interactions and communication. Using a powered wheelchair entails many important skills including self-navigation, spatial judgement, and being aware of the environment, and it requires a high level attention in order to drive safely. Children need to be able to practice in a safe and controlled environment with adult supervision. With the development of simulatorbased learning and virtual reality, children have the potential to benefit from these advances. Although powered mobility simulators have been developed and tested in a variety of different research studies, the evidence regarding their effectiveness is insufficient, and none are yet available for commercial practice.The overall aim of this dissertation was to examine powered mobility for children with physical disabilities as a prototype for gathering research and clinical evidence that to substantiate changes in healthcare policy, in this case decisions related to the provision of powered mobility. This was accomplished by carrying out a series of studies that addressed the following key questions: 1) Are there variables that predict a child's ability to drive a powered wheelchair proficiently? 2) Are pediatric powered mobility outcome measures, previously used for children aged 18 months to 6 years, reliable and valid for older children (6 -18 years)? 3) How can an adult-based simulator be modified for the use of children with physical disabilities? and 4) Is simulator-based powered mobility practice as effective as conventional powered mobility practice as measured by reliable and valid outcome measures? The results of each study were used to formulate changes in healthcare policy as discussed in the final sections of this dissertation.Article 1 is a retrospective study that examined the ALYN Hospital's powered mobility lending program since its inception in 2009. Data retrieved from children that participated from 2009-2016 were analyzed. The study compared demographic and functional powered mobility variables reported at the initial evaluation of 80 children to their driving abilities recorded at the end of the loan period. Three variables were found to predict with 80% accuracy whether a child would pass or fail a powered mobility proficiency test at the end of the program: 1) ability to propel a manual wheelchair for short distances, 2) ability to go-stop voluntarily on command and 3) using a joystick as an access mode. Children who controlled a powered wheelchair with alternative access modes (switches, scanner) often needed additional practice time, or did not become proficient at all. The results of this study were presented to the Israel Ministry of Health (IMOH) and resulted in significant changes in the age and level of demonstrated a priori (prior to training) proficiency needed for chair procurement.The outcome measures chosen for the different studies (PMP, PM-PT and the ALP) are acceptable outcome measures used in pediatric powered mobility for children 18 months to 18 years although they lack reliability and validity for children over 6 years of age. Article 2 describes the process used to determine the reliability and validity of the PMP, PM-PT and ALP for children, aged 6-18 years, with physical disabilities who were not proficient powered wheelchair drivers. The results of a study of 30 children showed that the PMP and PM-PT have very good intra- and inter-rater reliability and that the ALP has very good inter-rater reliability. All three outcome measures demonstrated convergent validity, with the PMP and PM-PT having higher levels of convergent validity than the ALP.Article 1 showed that in certain instances additional practice time might facilitate powered mobility proficiency. In some cases, additional practice time is not always possible for children with physical disabilities due to a shortage of practice powered wheelchairs or availability of therapist/caregiver to supervise practice. Alternate practice methods that are less dependent on the above barriers needed to be assessed. In Article 3, an adult powered mobility simulator was investigated to assess whether it was appropriate for children with physical disabilities. A focus group was conducted to identify the key factors of a powered mobility simulator for children. The main themes that emerged were that the simulation needed to be as realistic as possible, i.e., without animations or cartoon-like graphics and that the user should have a first-person perspective while interacting within the simulator and that the access mode should be a conventional powered wheelchair joystick.Based on the focus group results, the McGill Immersive Wheelchair Simulator (MiWe) was chosen to be adapted for children.
