Aims and Hypothesis: Tooth retention is important to oral health and life quality. However, recent trend suggest that clinicians increasingly extract teeth that could be treated and retained. Currently, no studies have quantified the changes in decision making for tooth extraction. Therefore, the present study was conducted to investigate whether there has been a change of criteria for first molar extraction at Tufts University School of Dental Medicine (TUSDM) from 2007 to 2018. It was hypothesized that the criteria for first molar extraction have changed toward extraction with less severe clinical conditions.Material and Methods: To test this hypothesis, a retrospective review of electronic records (Axium) was conducted among patients who had first molars extracted at TUSDM from 2007 to 2018. Interproximal bone level of one tooth per patient (N = 1200) was measured on radiographs. Clinical parameters and primary reasons for extractions were obtained from patient records. Data were analyzed by year and by three year-cohorts: 2007-2010, 2011-2014, and 2015-2018. Sample size of 100 teeth per year was determined from a pilot study measuring 20 teeth per cohort. Descriptive and analytical statistics were adjusted for potential effects of the confounders age, sex, and arch location.Results: The mean (SD) age of all subjects was 54.84 (15.93). The ratio of male: female was 1.37: 1. The ratio for arch location of extracted teeth for maxilla: mandible was 1: 1.16. The radiographic lowest bone level of extracted teeth increased significantly by year-cohort (P < 0.001) and by year (P < 0.001) with 1.2% annual increase after adjusting for patient age, sex and arch location. There was a decrease in the odds of having furcation involvement (FI) grade II when analyzed by year (OR = 0.913, P = 0.013) and by year-cohort (P = 0.036). Concurrently, prevalence of FI grade I increased across year-cohorts (P = 0.032). Teeth with a need for root canal treatment were increasingly extracted both by year (P = 0.001) and year-cohort (P = 0.002) analyses. Extraction for prosthodontic reasons increased (P = 0.024) while extraction for periodontal reasons decreased significantly by year (P = 0.002) and by year-cohorts (P = 0.045 and 0.004, respectively). On the other hand, there were no statistically significant differences in probing depth, clinical attachment level, tooth mobility, tooth restorability, and presence of root canal treatment of extracted teeth from 2007-2018.Conclusion: The results from this study point to significant changes in the criteria for tooth extraction from 2007-2018.
