Purpose: The purpose of this systematic review was to determine which inhaled pulmonary vasodilator (inhaled nitric oxide or inhaled prostacyclin) was superior in reducing pulmonary artery pressures (PAPs) in patients undergoing general anesthesia (GA).Methodology: An in-depth literature search was completed on peer-reviewed journal articles published between January 2000-August 2020. The electronic databases searched include EBSOhost, CINHAL, Science Direct, MEDline, and ProQuest. Google Scholar was also utilized to identify additional studies. Results: The primary outcome measured was mean PAP. These studies also included secondary outcome measures that included hypoxemia, central venous pressure (CVP), pulmonary vascular resistance (PVR), cardiac index (CI), cardiac output (CO), and mixed venous oxygenation (SvO2). The results showed that, when comparing iNO and PGI2, there was no significant difference in PAP in patients undergoing GA. The comparisons revealed similar results in reduction of PAP and PVR. The researchers failed to find consistent evidence to support a change in practice regarding the intraoperative management of pulmonary hypertension.Implications for Practice and Research: The finding of this systematic review did not support a change in practice. The decision to use either iNO or PGI2 should be based on multiple factors including anesthesia provider preference, facility training, and cost. More rigorous studies such as randomized control trials (RCTs), should be conducted with large, diverse samples comparing the intraoperative use of iNO and PGI2 to treat pulmonary hypertension.
