A systems approach to safety has the potential to significantly reduce patient harm events (Botney, 2008; Fowler, 2013; Wachter, 2010). However, safety professionals still struggle to develop intervention strategies that reflect substantive systems changes. To help remedy this issue, Wood and Wiegmann (2020) offered a formal definition of "systems change" and identified four system dimensions along which an intervention strategy can be evaluated to determine the level of "systems change" they reflect. A systems change hierarchy and associated rubric (SCR) for determining the extent to which a specific intervention strategy reflects a "systems change" was also provided.However, the SCR has yet to be evaluated for sources of validity or reliability evidence. The objective of this dissertation was to further the development and validation of the SCR. This objective will be met by conducting a sequential exploratory mixed methods study to meet the following aims: 1) Gather feedback from subject matter experts (SMEs) on the content and usability of the SCR, and 2) Determine the extent of inter-rater reliability that can be achieved by novices using the (revised) SCR to assess safety recommendations/intervention strategies.Aim 1 was met through the completion of an SME validation study, where SMEs were interviewed regarding the SCR for aspects of face validity, content validity, utility, and usability. Guided by these frameworks, a deductive content analysis (Neuman & Kreuger, 2000) of SME responses was conducted. This analysis identified necessary rubric revisions that culminated into a revised version of the SCR (SCR 2.0). Aim 2 was met by conducting an inter-rater reliability study with novice users applying SCR 2.0 to a series of patient safety case studies.These efforts will inform and support healthcare professionals' approach to developing systems changes needed to transform healthcare and improve patient safety.
