Emergency department (ED) providers are expected to deliver timely, life-saving interventions in what is often a chaotic and stressful environment. In addition to the high number of medications ordered and prescribed in the ED, physicians and nursing staff are expected to make life-saving decisions regarding medications within seconds during resuscitations and traumas. A valuable resource for the emergency healthcare team is the addition of a dedicated clinical pharmacist. This Sacramento area emergency department is one of the busiest emergency departments in the State of California. The lack of a clinical pharmacist is associated with an increased risk for harm to patients and stress on the staff. It also represents a significant missed opportunity to improve care while generating a cost-reduction through the value-added interventions that clinical pharmacists provide. The aim of this project was to assess the needs of the Sacramento area hospital's ED and create a setting-specific business case supporting the implementation a dedicated clinical pharmacist by examining the cost-benefit analysis, the effect on nurse and physician satisfaction, and the influence on medication safety. This was accomplished by completing three objectives: 1) draft a business case proposal for an ED clinical pharmacist based on the literature and existing data, 2) obtain reviews of the proposed business case from informed experts in the field, and 3) revise and finalize the business case according to the feedback provided by the panel of experts. The final business case was presented to the hospital leadership in September 2018 for consideration as part of the department expansion occurring in 2020.
