The challenge to translate research efficacy into primary care providers' standard clinical care of patients suffering with back pain is fraught with obstacles. Issues such as the shortage in primary care providers, competing quality care initiatives, the introduction of electronic health records, and the public demand for opioid prescriptions are some of the encountered challenges. This case study of education and implementation of a clinical decision support tool in a primary care setting gave an opportunity to understand factors the primary care staff and clinicians perceived as important to the success of implementing and using a decision support tool. Collection and analysis of the data was guided by the Consolidated Framework for Implementation Research (CFIR). Populated by patient reported outcome information, the decision support tool incorporated an alert reminding the clinicians to order physical therapy services for patients. The role of the primary care staff within the practice influenced perceptions of success, and included changes in perceptions concerning technology limitations and patient participation. The primary care practice staff's tenure with the practice did not influence changes in perceptions. The introduction and use of the decision tool did result in increased prescribing of physical therapy services for patients suffering with back pain. These results encourage continued research efforts surrounding incorporation of education and technology uses into the practical care of patients and the importance of understanding translation obstacles and factors of implementation success.
